MyChart accounts are available to Rush-Copley patients age 18 or older. If you are under the age of 12, your parent or guardian can access your record through their MyChart account after submitting a Child Proxy Access Consent Form to your healthcare provider's office or the Health Information Management office.

Both outpatients and inpatients can access to MyChart by filling out an Access Request form. If you include your email address on the form, Rush will email your activation code. If you include your email address on the form, Rush will email your activation code.
